Booking an appointment

There are two ways to reach the clinic. Call (941) 921-2225 and the front desk can answer questions about your situation, or use the online booking link to request a time that works for you. If you were recently in a car accident, mention that when you call so your visit can be scheduled appropriately.

Parking is available at the Bee Ridge Road office, and the map alongside this page will open directions in Google Maps.

What you may want to bring

• A photo ID
• A current list of your medications
• Any imaging reports or records relevant to your symptoms
• Your insurance information, and the crash report if you have one

Please do not send sensitive medical, insurance, or identifying details by email or through a general web form. Bring those details to your visit or discuss them by phone.
